Job summary

Saint Peter's Health Care System is seeking a Social Work Manager for Care Coordination in New Jersey. You will supervise Social Work staff and ensure integration of services across psychosocial, financial, and behavioral health aspects of care, discharge planning, and transitions in care.

The role requires an MSW with NJ LCSW licensure, clinical supervision credentials, and at least five years of related experience, preferably in acute care.

Qualifications

• Master's degree from an accredited school of social work is required.
• NJ licensure as an LCSW is required.
• NPI and Medical Non-Billing Provider Number are required; apply within 30 days of employment.
• Clinical Supervision Certificate required to provide supervision for LSW.
• Certified EARC Trainer-DoH or within six months of hire.
• At least five years of social work and discharge planning experience; acute care hospital preferred.
• Managerial experience preferred.
• Knowledge of psychosocial assessment, case management, discharge planning/transitions in care, and related regulations.
• Excellent written and oral communication and interpersonal skills; strong planning and organizational skills.
• Strong critical thinking and analytical abilities for complex patient situations.

Responsibilities

• Manage daily operations and supervision of Social Work staff.
• Integrate Social Work services into departmental and organizational care systems.
• Oversee psychosocial, financial, behavioral health aspects of care and transitions in care.
• Manage hospital ambulance transportation contract operations.
• Lead behavioral health and ethical issues within the department.
• Collaborate with the Care Coordination Nursing Manager to meet departmental goals.
