Job summary

Maple Glen Center in New Jersey is seeking a Social Services Specialist to identify the social, emotional, and mental health needs of patients and families and arrange services to enhance well-being. The role emphasizes NASW ethics and compliance with regulations while delivering high-quality care.

You will work with an interdisciplinary team to develop care plans, hold meetings with families, and coordinate discharge planning and community referrals to support residents' quality of life.

Qualifications

• Bachelor's degree in social work or human services field required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Enrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs.
• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
