Qualifications

• Two years of any combination of experience working with communities, health or social service matters, social work, case management, behavioral health, or health care, preferably with individuals with intellectual and developmental disabilities.
• Associate's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology, Family Studies/Services, Social Sciences, or Human Services and one year of experience as a social work assistant and/or in social or community coordination; three years may substitute for the required education/experience.
• Must possess and maintain a valid New Mexico Driver's License.

Responsibilities

• Provide guidance, technical assistance, and outreach to individuals, families, providers, and community partners to promote person-centered planning, meaningful community inclusion, and competitive integrated employment.
• Support participants in achieving their individual goals by fostering inclusive community supports and opportunities that enhance independence, well-being, and quality of life.
• Conduct in-person visits with individuals enrolled in DDSD waiver programs to assess health, safety, satisfaction, and overall well‑being.
• Collaborate with internal and external stakeholders to advance Employment First principles and promote school-to-work transitions for eligible individuals.
• Promote community inclusion and employment initiatives through outreach with schools, providers, employers, and community organizations.
