Job summary

Hearts of Hope, LLC in New Mexico seeks a dedicated Service Coordinator to develop, implement, and monitor person-centered services for individuals supported under the DD Waiver. You will work directly with individuals, families, DSPs, supervisors, therapists, and IDT members to ensure regulatory compliance, quality documentation, and goal achievement while promoting health and safety across your caseload.

Reports to the Office Manager/Agency Director and participates in ongoing DDSD trainings

Qualifications

• High School Diploma or GED required; higher degree preferred in human services.
• Valid NM driver's license and reliable vehicle for client transport.

Responsibilities

• Develop and monitor Individual Service Plans (ISPs) and assessments.
• Provide guidance to Direct Support Professionals and supervisors.
• Ensure regulatory compliance and accurate documentation (MARs, IRs, audits).
• Coordinate with IDT and external providers to advocate for individuals’ rights and community inclusion.
• Participate in on-call rotation and maintain organized electronic records.
