Job summary

The State of Missouri Department of Health and Senior Services is seeking a qualified professional to assess and coordinate Home and Community Based Services for seniors and adults with disabilities. The role emphasizes thorough assessments, care planning, and multi-county home visits.

Responsibilities include client and provider communications, service authorizations, and representing the department in administrative hearings.

Qualifications

High level communication and interviewing skills.
Able to work independently and show initiative within a team environment.
Able to be resilient, with critical and innovative thinking skills
Bachelor's degree or higher level degree (RN/LPN or related fields may substitute for education).
RN & LPN must be licensed in Missouri.

Responsibilities

Conduct initial assessments/reassessments to determine least restrictive environment for seniors and disabled adults.
Coordinate Home and Community Based Services (HCBS) by contacting participants and providers for service authorizations.
Develop and implement care plans meeting unmet needs, including home visits and provider options.
Travel within a multi-county area to conduct home visits, hearings and trainings using personal vehicle when needed.
Ensure program integrity, operations and overall success.
Represent DSDS in administrative hearings.
