Job summary

The Missouri Department of Health and Senior Services seeks a dedicated professional to assess and coordinate Home and Community Based Services for seniors and adults with disabilities in Joplin. This role supports independence, teamwork, and compassionate care while promoting health, safety and wellbeing.

Responsibilities include conducting assessments, developing care plans, coordinating services, traveling within a multi-county area, and representing DSDS in administrative hearings.

Qualifications

• Bachelor’s degree or higher in a relevant field or equivalent experience.
• RN/LPN license in Missouri in good standing; applicable substitutions allowed for years of education.
• Experience with elder or disabled populations or related human services preferred.

Responsibilities

• Conduct initial assessments/reassessments to enable seniors and/or disabled adults to remain in the least restrictive environment.
• Coordinate HCBS program services by contacting participants and providers to complete service authorizations.
• Develop and implement care plans addressing participant needs and provider options.
• Travel within a multi-county area to conduct visits, hearings, and trainings.
• Maintain program integrity, operations, and accountability; represent DSDS in administrative hearings.
