Access to Healthcare Network (AHN) seeks a Senior Services Social Worker I in Las Vegas to support older adults and individuals with disabilities through transition planning, care coordination, and resource navigation within the Money Follows the Person (MFP) program. You will partner with providers and housing partners to promote independence after discharge from facility care.

Qualifications

• Bachelor's degree in social work, Human Services, Psychology, Gerontology, Public Health, or related field required.

Responsibilities

• Conduct comprehensive assessments to identify participant strengths, needs, and goals related to community living.
• Develop and implement person-centered transition plans with participants and family members.
• Coordinate services for community reintegration including healthcare, behavioral health, transportation, housing, and long-term supports.
• Maintain regular contact with participants and monitor progress.
• Document assessments, service plans, referrals, and outcomes in required records.
• Follow HIPAA and program requirements.
