Access to Healthcare Network (AHN) seeks a Senior Services Social Worker I to support seniors and people with disabilities in transitioning from facilities to community living under the Money Follows the Person (MFP) program. You will plan, coordinate, and monitor person-centered transitions in partnership with facilities, providers, and housing partners.

Responsibilities include comprehensive assessments, care coordination, benefit navigation, and education on rights and supports, with a focus

Qualifications

• Bachelor’s degree in social work, human services, psychology, gerontology, public health, or related field.
• Two years of experience in case management, care coordination, discharge planning, or related fields.
• Spanish–English bilingual preferred; Medicare Assistance Program Level II Certification within 2 months of hire.

Responsibilities

• Conduct comprehensive assessments to identify participant strengths, needs, goals.
• Develop and implement person-centered transition plans with participants and partners.
• Coordinate services for community reintegration including healthcare, housing, and transportation.
• Maintain regular contact with participants pre- and post-transition; monitor progress.
