Job summary

Access to Healthcare Network (AHN) seeks a Senior Services Social Worker I in Las Vegas to support older adults and people with disabilities in navigating services and securing independent community living through the Money Follows the Person program.

You will partner with healthcare providers and community partners to develop transition plans, coordinate services, and follow up to ensure safe, successful community reintegration, while documenting outcomes and maintaining HIPAA-compliant records.

Qualifications

• Bachelor's degree in social work, human services, psychology, gerontology, public health, or related field is required.
• Minimum of two years of experience in case management, care coordination, discharge planning, social services, aging services, disability services, healthcare, or behavioral health.

Responsibilities

• Transition Planning & Coordination: conduct assessments, develop transition plans, coordinate needed services, and ensure continuity of care.
• Case Management & Support: maintain ongoing contact, monitor progress, address barriers, assist with benefits and resources, provide education.
