Job summary

Qualifications

• High school diploma or equivalent required; college degree preferred.
• 2+ years in community outreach or related roles.
• Ability to build trust with community organizations and leaders.
• Strong verbal communication and public speaking skills.
• Ability to work evenings and weekends as needed.
• Basic computer/data entry proficiency.

Responsibilities

• Establish and maintain relationships with community and faith-based organizations.
• Represent St. John’s Community Health at events, health fairs, outreach activities, and partner meetings.
• Recruit and engage seniors who may benefit from healthcare and supportive services.
• Conduct presentations about program services, eligibility requirements, and participation opportunities.
• Guide seniors and families through intake, enrollment, and service access processes.
• Follow up with prospective participants and address barriers to access such as transportation, scheduling, language access, and documentation.
• Track outreach metrics and report to leadership.
• Identify emerging community needs and advocate for service improvements.
