Job summary

Eskaton is a nonprofit aging leader in Northern California seeking a social service professional to provide general case management, intake, and referrals for residents needing support.

You will establish community linkages, educate residents about available services, and monitor ongoing care, helping build informal networks to support aging in place. The role emphasizes advocacy and compassionate service.

Qualifications

• Education requirements include a Bachelor of Social Work or related field (Gerontology/Psychology/Counseling).
• Two years of progressive experience in social service functions.
• Knowledge of supportive services for seniors; ability to advocate, organize and problem-solve.

Responsibilities

• Provide general case management and referrals to residents.
• Establish linkages with agencies and service providers; develop best-priced options.
• Set up a provider directory for staff and residents.
• Educate residents on service availability and procedures; provide advocacy.
• Monitor ongoing services and maintain confidential records; ensure compliance with policies.
• Help residents build informal support networks and coordinate volunteer programs.
• May train residents on tenancy obligations and coordinate related activities.
• Collaborate with assessment services or PAC to develop case plans.
• Follow safety guidelines and Eskaton policies in daily work.
