Job summary

National Church Residences in Xenia, OH is seeking a Service Coordinator for John Sale Manor. This full-time role coordinates programs and services to help seniors age in place, following participant-centered, non-clinical assessment and documentation practices.

You will liaise with community agencies, develop a local resource directory, organize health and life-skills events, and support residents with transportation, meals, and personal services.

Qualifications

• High school diploma or GED required; bachelor’s degree in social work or related field preferred.
• Two or more years of experience in social services with elderly, disabled, or families.
• Good communication and interpersonal skills; working knowledge of Windows and MS Office.

Responsibilities

• Coordinate programs and activities for eligible individuals.
• Liaise with community agencies; identify low-cost services and negotiate discounts.
• Document services in the management system and maintain the Resource Directory.
• Organize educational events on health care, agency support, life skills, and referrals.
• Identify partners to provide health services and screenings; engage volunteers.
