Job summary

The Lehigh County Area Agency on Aging is hiring an Aging Case Aide 2 to assist older adults and their families in accessing services that promote independence and well-being. You will perform intake, referral tracking, and person-centered counseling to help clients choose appropriate options and complete applications.

Responsibilities include documenting case histories, coordinating with community resources, and following up to ensure needs are met.

Qualifications

• One year as an Aging Case Aide 1; or two years of public contact work, including one year in a human services program.
• Any equivalent combination of experience and training.
• PA residency required.

Responsibilities

• Referral tracking using computer systems to enter data and monitor referrals.
• Intake processing by interviewing consumers to assess needs and eligibility for aging services.
• Person-centered counseling to help weigh options and plan actions.
• Case follow-up with consumers and referral sources to ensure needs are met.
• Maintain detailed case histories, notes, memos, and reports.
• Participate in community events and in-service training to increase knowledge.
