Job summary

Serving Seniors seeks an HSS Case Manager - Homeless Services at the Wellness Center in San Diego. You will provide individualized social services, health referrals, and support to older adults, coordinating with partners and maintaining documentation.

The role requires a bachelor’s degree or equivalent experience; bilingual ability is a plus. A full benefits package includes paid medical/dental, holidays, and a retirement match.

Qualifications

• Bachelor’s degree preferred or equivalent combination of education and experience sufficient to perform the key accountabilities.

Responsibilities

• Connects clients to in-house and community resources.
• Liaises between clients and community providers, including referrals to Health Educators.
• Updates and manages community resources listings.
• Provide excellent customer service in an inter-disciplinary setting.
• Advocate and outreach to the community and stakeholders.
• Manage caseload with walk-ins and appointments.
• Develops and implements care plans for unmet needs with referrals.
• Conducts Universal Senior Assessment as applicable.
• Represent Serving Seniors at community meetings for older adults.
• Document all efforts and data entry in Case Worthy/Teams.
