Job summary

Serving Seniors is seeking an HSS Case Manager to support adults 55+ with individualized social and health services, referrals to partners, and guidance for healthy daily living. The role emphasizes outreach, coordination with a multidisciplinary team, and compassionate advocacy at the Mary & Gary Wellness Center.

The role requires a bachelor’s degree or equivalent and may prefer bilingual Spanish. This is a full-time, on-site position with opportunities to impact seniors across San Diego County.

Qualifications

• Bachelor’s degree preferred or equivalent combination of education and experience sufficient to successfully perform the key accountabilities of the job.
• Bilingual Spanish preferred

Responsibilities

• Connects clients to both in-house and community resources.
• Provides information on community health and welfare resources and acts as a liaison between community service providers and clients.
• Updates and manages community resources listings.
• Adheres to established policies and procedures, provides excellent customer service, and demonstrates ability to work in an interdisciplinary setting.
• Represents Serving Seniors at community meetings specific to older adults.
