Job summary

Serving Seniors is seeking a Health & Social Services (HSS) Case Manager to support older adults 55 and over at the Mary and Gary Wellness Center in San Diego. The role requires independent and collaborative work with a multi-disciplinary team to deliver individualized social services, health referrals, and daily living support.

The position involves outreach at the Wellness Center, familiarity with CES and HMIS, and ensuring safety through Client Code of Conduct education.

Qualifications

• Bachelor’s degree preferred or equivalent combination of education and experience sufficient to perform key duties.
• Excellent analytical, verbal, and written communication skills; ability to establish rapport.

Responsibilities

• Connects clients to in-house and community resources.
• Acts as liaison between community service providers and clients, including referrals to health educators.
• Updates and manages community resources listings.
• Provides advocacy and outreach to the community and stakeholders.
• Manages a caseload with walk-in and appointment hours.
• Documents client interactions and data entry accurately in software systems.
