Job summary

You Thrive Florida in Leesburg, FL is seeking a dedicated Case Manager to coordinate services for functionally impaired older adults. You'll assess needs, connect clients with community resources, and ensure continuous care aligned with plans.

Responsibilities include home visits, care plan reviews, documenting case notes, and collaborating with staff to promote client self-sufficiency while maintaining confidentiality and compliance with regulations.

Qualifications

• Bachelor’s Degree in Human Services or related field and one year of Case Management experience.
• Valid Florida Driver’s license and be insurable by the company’s current insurance carrier.
• Subject to federal, state and local legal requirements/background checks/clearance for working with vulnerable populations.
• Complies with federal, state and local regulations applicable to program sources.

Responsibilities

• Coordinate services using client-centered principles to support self-sufficiency.
• Review Care Plans and conduct home visits to perform semi-annual/annual reviews.
• Assess client safety through calls or home visits as required and address needs.
• Maintain and update client case records to ensure timely, quality documentation.
• Participate in staff meetings and community outreach activities.
