Job summary

OCES is expanding to better serve the South Shore, including Quincy, Braintree, Weymouth and surrounding towns. We are seeking a collaborative, mission-driven coordinator to connect residents with essential information and services.

The role emphasizes team leadership, care conferences, and program planning to maintain active, engaged communities for older adults and individuals with disabilities.

Qualifications

• Two years of experience in the human services field, preferably with the elder population.
• Excellent written and verbal communication skills.
• Ability to adapt to changing consumer and program needs.

Responsibilities

• Review residents' needs to link community programs and services.
• Serve as a multi-disciplinary assessment team leader and meet with the team to assure service coordination.
• Facilitate care conferences with residents, family, and other participants.
• Plan and coordinate social, educational, and recreational programs for residents.
• Prepare and distribute a newsletter to keep residents informed of planned activities.

Education

Bachelor's degree in social work or related field
