Job summary

Old Colony Elder Services (OCES) is expanding to cover the South Shore and seeks a Covering Care Manager to provide immediate support when the assigned manager is unavailable. You will respond to calls, document actions, and help maintain continuity of care for older adults and individuals with disabilities.

This hybrid role offers a supportive team, professional development opportunities, a 35-hour week, and the chance to conduct home visits as needed.

Qualifications

• Minimum of one year of experience in the human services field.
• Excellent written and verbal skills; ability to document actions clearly.
• Proficiency with Microsoft Office Suite or related software.
• Ability to adapt as program needs change and to work on multiple teams.

Responsibilities

• Respond to inbound calls for designated teams and provide coverage as needed.
• Take follow-up actions for uncovered caseloads and ensure timely responses.
• Document actions, communications, and interventions in the database accurately.
• Assist in developing and reviewing care plans for participants.
• Complete calls to providers and perform follow-up actions, including authorizations.
• Conduct ongoing calls to consumers as directed; may include home visits.
