Job summary

Old Colony Elder Services (OCES) is seeking a Home Care Program Manager to plan, organize, manage and evaluate Home Care program operations and staff. The role emphasizes supervising assigned staff, overseeing care management, and ensuring regulatory compliance within a hybrid work environment.

The position offers opportunities for professional development and a supportive team, with focus on delivering compassionate care to older adults and individuals with disabilities in the south shore area.

Qualifications

• Minimum of five years of experience in the human services field; including two years in a supervisory capacity.
• Excellent written and verbal skills.
• Excellent computer skills; proficient with Microsoft Office Suite or related software.

Responsibilities

• Provide oversight and direction to assigned staff.
• Oversee all care management aspects of HC program.
• Contribute to development and implementation of procedures, standards and guidelines.
• Ensure program integrity and regulatory compliance.
• Implement quality measures and conduct quality reviews.
• Plan and execute comprehensive staff development and training programs.
• Analyze program evaluation including service needs/gaps and quality/effectiveness of services.
• Advise and consult with other HC management staff regularly.
• Represent OCES at external meetings and in public outreach/education activities.
• Maintain knowledge of OCES policies, standards, and funding source regulations.
• Maintain accurate documentation.
