Job summary

Consumer Direct Care Network is seeking a Service Coordinator Supervisor to oversee Medicaid client caseloads and coordinate in-home care services in compliance with regulations. The role includes leadership, staff training, and collaboration with stakeholders to ensure quality service delivery.

The position requires a bachelor’s degree in a related field, 3 years of relevant experience, and the ability to manage multiple priorities and special projects.

Qualifications

• Bachelor’s degree preferred in social work, psychology, counseling, rehabilitation, nursing, sociology or related field.
• 3 years experience working with individuals with disabilities or elderly preferred.
• Combination of education and experience considered.
• Leadership experience preferred.
• Ability to work flexible and/or extended hours, if needed.

Responsibilities

• Maintain a caseload of clients enrolled in Medicaid programs.
• Supervise in-home care needs in line with Federal/State Regulations and MCO rules.
• Oversee intake and referral processes to support growth.
• Provide leadership and training to other staff.
• Collaborate with stakeholders on service delivery.
• Lead, supervise and complete special projects including large-scale enrollments and transitions.
• Market services to referral sources.
• Coordinate client assessments and home visits.
