Job summary

Consumer Direct Care Network is seeking a Service/Program Coordinator to manage a Medicaid caseload and coordinate in-home care services in compliance with federal and state regulations. You will assist with intake, referrals, and maintaining care plans while delivering excellent customer service to clients and providers.

The role involves coordinating with stakeholders, performing client assessments, and enrolling clients in services and EVV portals.

Qualifications

• Bachelor’s degree preferred in a related field.
• 1 year of experience with individuals with disabilities or the elderly preferred.
• Combination of education and experience accepted.
• Remote/hybrid positions require reliable internet.

Responsibilities

• Maintain and coordinate a caseload of clients enrolled in Medicaid programs.
• Coordinate in-home care needs in compliance with Federal/State regulations and MCO rules.
• Assist with daily intake and referral processes to support business growth.
• Provide excellent customer service to internal and external clients.
• Represent the company at stakeholder meetings and events.
• Enroll clients and employees in services, portals, and EVV systems.
• Conduct client assessments and home visits as required.

Education

Bachelor’s degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or Related Field
