Job summary

21st Century Home Health Services is seeking a Registered Nurse Case Manager Level 2 to serve Woodland and surrounding areas. You will oversee the comprehensive plan of care and manage SOCs, evaluations, and discharge planning for home-based patients.

You will supervise two nurses, coordinate with physicians and staff, and mentor teams to deliver high-quality outcomes, while ensuring compliance and patient education.

Qualifications

• CA RN license required and valid.
• BLS/CPR certification.
• Valid CA driver's license.
• Minimum 1 year home health case management; 2+ years preferred.
• Experience supervising nurses is highly preferred.
• Strong leadership and collaboration across disciplines.

Responsibilities

• Oversee care plans; manage SOCs, evaluations, and discharge planning.
• Supervise two nurses; ensure quality and compliance.
• Collaborate with physicians and agency staff.
• Develop and implement plans of care in patients' homes.
• Provide education to patients and families.
