Job summary

CVS Health in Bronx, NY seeks an experienced RN Case Manager to lead care management activities, drive care coordination, and collaborate with interdisciplinary teams to ensure continuity of care for complex patients. You will manage a caseload, own care coordination, oversee transitions of care, document in the EHR, and participate in interdisciplinary planning.

Requirements include RN license in the state, 6-8 years nursing experience, CCM certification or willingness to obtain, and bilingual

Qualifications

• RN license in assigned state required; Bachelor degree in nursing preferred.
• CCM certification required or willingness to obtain within 12 months of hire.
• 6–8 years nursing experience; 2+ years in transitional nursing, care coordination, discharge planning, or home health preferred.

Responsibilities

• Manage an assigned caseload of complex patients in a value-based care environment.
• Accountable for panel metric performance in admission prevention and readmission prevention.
• Own overall care coordination for assigned patients as the primary point of contact.
• Manage transitions of care episodes, including clinical assessments and medication reconciliation.
• Collaborate with patients' PCP, family, Social Workers, and other care team members to evaluate needs.
• Ensure timely documentation of key clinical assessments after admissions.
• Lead in-person interdisciplinary care planning meetings.
• Perform timely nursing assessments and provide patient education for chronic condition management.
• Educate patients and families, empowering them in their care.
• Document visits in electronic health records according to internal standards.
