Job summary

Qualifications

• CCM certification required or willingness to obtain within 12 months of hire.
• 6–8 years nursing experience; 2+ years in transitional nursing, care coordination, discharge planning, or home health preferred.

Responsibilities

• Manage an assigned caseload of complex patients in a value-based care environment.
• Accountable for panel metric performance in admission prevention and readmission prevention.
• Own overall care coordination for assigned patients as the primary point of contact.
• Manage transitions of care episodes, including clinical assessments and medication reconciliation.
• Collaborate with patients' PCP, family, Social Workers, and other care team members to evaluate needs.
• Ensure timely documentation of key clinical assessments after admissions.
• Lead in-person interdisciplinary care planning meetings.
• Perform timely nursing assessments and provide patient education for chronic condition management.
• Educate patients and families, empowering them in their care.
• Document visits in electronic health records according to internal standards.

About This Role

The RN, Case Manager at CVS Health is responsible for leading care management activities, driving care coordination, and collaborating with interdisciplinary teams to ensure continuity of care for complex patients. What You'll Do

Requirements

• Experience: Minimum of 6-8 years nursing experience. 2+ years experience in transitional nursing, emergency room nursing, care coordination, discharge planning, or home health preferred.
• Licenses & Certifications: Certified Case Manager (CCM) required, or willingness to obtain within 12 months of hire. Fluency in Spanish or other languages spoken by the communities served is required.
