Excellus BCBS in New York is seeking a Case Management professional to conduct program activities aligned with CMSA and NCQA accreditation standards. You will identify eligible members, educate and coach them, coordinate care with providers, and implement cost-effective, high-quality plans.

Work with an interdisciplinary team to develop comprehensive care plans, monitor outcomes, and adjust care as needed. Strong clinical experience, regulatory knowledge, and MS Office proficiency are essential.

Qualificações

• Associates degree required; bachelor’s degree preferred.
• Active NYS RN or Registered Dietician or Physical Therapist licensure required.
• Minimum of three years of clinical experience; Case Management experience preferred.
• Proficient in Microsoft Office Suite.

Responsabilidades

• Handles CMSA/NCQA standards compliance within case management activities.
• Develops comprehensive care plans with interdisciplinary care team.
• Coordinates services and resources to meet member needs and goals.
• Mentors junior staff and supports coaching where needed.
• Ensures privacy and regulatory compliance in all activities.
• Collaborates with physicians and providers to optimize member outcomes.
