Job summary

Qualifications

Responsibilities

• Provide competent nursing care by executing job responsibilities safely and consistently.
• Respond to incoming telephonic requests and ensure responsiveness to patient needs.
• Provide clinically competent triage and symptom management to patients.
• Utilise standardized protocols for medication management and prescription refills.
• Review patient medications and update as needed.
• Offer education and direct patient care around chronic conditions.
• Collaborate and monitor care plan implementation for patients.
• Build relationships with patients and caregivers to support preventative care.
• Create a welcoming environment for patients, communities and teams.
• Participate in care team meetings to discuss patient care and clinic operations.
