Job summary

Northwest Texas Healthcare System in Amarillo, TX is seeking a Case Manager (RN) to lead discharge planning and coordinate patient care with the treatment team. The RN will assess patient needs, collaborate with social workers, and conduct utilization management and insurance reviews as required by managed care providers.

Qualified candidates are registered nurses licensed in Texas with a Bachelor’s degree and at least five years of clinical experience, plus current BLS certification.

Qualifications

• RN licensed in Texas with a Bachelor's Degree (in nursing or equivalent) and minimum five years of clinical experience.
• Excels in interpersonal and communication skills.
• Current BLS certification upon hire or within 30 days and maintain throughout employment.

Responsibilities

• Provides leadership in departmental services through collaboration across NWTHS.
• Assesses patient healthcare and education needs with the healthcare team to meet those needs.
• Coordinates hospital processes for seamless, efficient care and allocates resources to meet patient needs.
• Manages utilization review and insurance reviews as indicated by managed care companies.
• Ensures compliance with education requirements and safety standards to minimize risk to staff.
