Aledade is seeking a Senior Post Acute Transition Specialist to drive post-acute care initiatives for our flagship Enterprise Practice and affiliated health system. You will serve as an executive-level Account Manager, forging partnerships with health system leadership and coordinating care across the local ecosystem.

Qualifications

• Bachelor's degree in a clinical or healthcare-related discipline is required.
• Minimum of 5+ years of clinical experience in a complex healthcare environment.
• Minimum of 6+ years in value-based care (ACOs, MCOs, or large health systems).
• Autonomy in spearheading complex strategic projects and managing executive stakeholders.
• Advanced ability to craft data-driven collateral to drive change and decisions.
• Experience collaborating with post-acute providers to optimize the care continuum.

Responsibilities

• Act as principal post-acute authority and liaison for the flagship Enterprise Practice.
• Develop data-driven collateral aligning claims data and clinical insights to workflows.
• Lead quarterly Town Halls aligning SNFs, hospitals and practice leads.
