Rimland is seeking a M-F Qualified Intellectual Disability Professional (QIDP) in Des Plaines, IL. The role focuses on coordinating services, monitoring client progress, and ensuring plan implementations align with individual goals.

You will advocate for clients, participate in care planning, and ensure DHS/HCBS compliance. The schedule is Monday–Friday, 9:00 AM–6:00 PM, with a base rate of $27.60/hour and overtime eligibility.

Qualificações

• 1+ year of experience working directly with individuals with intellectual or developmental disabilities.
• Bachelor's degree in a human services field as listed.
• Valid driver's license and clean driving history to drive company vehicles.
• Reliable transportation.

Responsabilidades

• Coordinate and monitor each client's progress towards ISP goals.
• Participate in all Personal Care Plan meetings and lead ISSA visits.
• Communicate safety and well-being issues to supervisors.
• Assist clients in the community to implement the Calendar of Life.
• Maintain good relationships with volunteers and community organizations.
• Ensure supplies are available to carry out identified outcomes per the plan.

Bachelor's degree in human services fields (social work, psychology, sociology, special education, or rehabilitation counseling)
