Job summary

Ray Graham Association in Elmhurst, IL, is seeking a Residential Qualified Intellectual Disabilities Professional (QIDP) to coordinate person-centered services for individuals with IDD. You will lead interdisciplinary teams, develop and monitor individualized plans, and ensure regulatory compliance with DHS, IDPH, DCFS, CMS, and other agencies.

The role emphasizes advocacy, strong collaboration with families, and ensuring high-quality supports that promote independence and meaningful lives.

Qualifications

• Bachelor's degree in Human Services, Psychology, Social Work, Special Education, or related field.
• QIDP Registry certification is required.
• Illinois DSP Registry status required.
• Valid IL driver's license with acceptable driving record.
• Ability to travel throughout DuPage County and other Ray Graham locations.
• Background checks and drug screening completed as part of pre-employment process.

Responsibilities

• Develop, implement, and monitor person-centered plans and behavior support plans.
• Lead interdisciplinary teams to create customized supports.
• Advocate for individuals and promote informed choice and independence.
• Coordinate medical, behavioral, and support services for continuity of care.
• Monitor plan implementation and address barriers to outcomes.
