Majestic Care of Saint Anthony is seeking a Physical Therapist to evaluate, plan, and implement individualized treatments for residents in a skilled nursing facility. The role focuses on improving mobility, strength, balance, and overall independence within a resident-centered care team.

The PT will collaborate with nursing, occupational therapy, and speech therapy to ensure coordinated care, maintain HIPAA compliance, and contribute to quality improvement in rehabilitation services.

Qualifications

• Current state licensure as a Physical Therapist.

Responsabilités

• Conduct initial and ongoing evaluations to assess residents' physical therapy needs, including mobility, strength, balance, and functional limitations.
• Develop and implement individualized treatment plans based on clinical findings, resident goals, and functional abilities.
• Provide direct physical therapy interventions such as therapeutic exercises, gait and balance training, mobility enhancement, and manual therapy.
• Utilize evidence-based practices to promote optimal functional outcomes and independence.
• Monitor and document residents' progress, adjusting treatment plans as necessary to reflect changes in condition or goals.
• Educate residents, families, and caregivers on treatment plans, home exercises, and safety strategies to support continued progress and prevent injury.
