Job summary

Qualifications

• Minimum 4–5 years of experience in gerontology or social services, plus 1–2 year post-secondary certification/diploma.
• Ability to identify and resolve issues using standard procedures and knowledge.
• Strong communication, diplomacy and empathy to interpret information and advise clients or stakeholders.

Responsibilities

• Assess client eligibility for PATH program according to policies.
• Onboard new clients by reviewing program offerings and costs.
• Research, develop, and implement new or realigned services/initiatives.
• Liaise with community to identify vulnerabilities and service gaps.
• Coordinate with community agencies to foster partnerships.
• Develop implementation plans for program delivery and monitor outcomes.
• Assist with budget preparation, reporting and financial tracking.
• Support management of premises, vehicles, security and petty cash.
