Job summary

Greenbrook Medical seeks a physician to join its senior primary care model serving Medicare Advantage patients in Florida. You will manage a panel of about 500 patients with 30–45 minute visits focused on prevention, chronic disease management, and healthy aging.

Responsibilities include coordinating care across teams, leveraging data for outreach, and reducing cost of care while delivering exceptional patient experiences. Competitive compensation and growth opportunities offered.

Qualifications

• MD or DO in Internal Medicine or Family Medicine
• Board certified or board eligible
• Active Florida medical license or ability to obtain
• Experience in primary care, managed care, value-based care
• Residency in Internal Medicine or Family Medicine
• Commitment to building relationships with patients

Responsibilities

• Deliver primary care focused on prevention, chronic disease management, and healthy aging.
• Build relationships with a panel of ~500 Medicare Advantage patients.
• Create personalized care plans to improve quality of life and health outcomes.
• Coordinate care with MAs, care managers, specialists, pharmacists, hospitals, and providers.
• Lead care plan discussions and ensure smooth transitions across care teams.
• Proactively identify care gaps to improve quality metrics and outcomes.
• Leverage Epic/eClinicalWorks and population health data for care coordination.
• Provide a concierge-like patient experience with empathy and education.
