This 100% outpatient role features a small patient panel with strong team support and access to in-house specialists, with no nights or weekends required. The base salary ranges from $221,141 to $315,915 annually, with a guaranteed salary and quality-based bonus opportunities.

Qualifications

• Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine REQUIRED

Responsibilities

• Serve as a true hands-on caregiver for senior patients
• Deliver high-touch, relationship-centered primary care within a value-based care model
• Work alongside a dedicated care and support team and in-house consulting specialists to improve patient health and well-being
