Job summary

Humana's Conviva Senior Primary Care in Wellington, Florida, is hiring a Nurse Practitioner or Physician Assistant to join a collaborative care team delivering value-based care to seniors. The role is onsite at the Conviva Wellington Family Center, reporting to the Associate Medical Director. Expect 40 hours per week with a competitive pay range and bonus eligibility.

Required: Master's in Nursing or PA program with board certification and active NP/PA license in-state. TB screening is required.

Qualifications

• Master's Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• TB screening requirement.
• Preferred: Experience in value-based care, primary care or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.
• Familiarity with HEDIS quality indicators and Medicare guidelines.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure high-quality, coordinated care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
