CenterWell Senior Primary Care in Biloxi, MS is seeking a Nurse Practitioner or Physician Assistant to join a collaborative, patient-centered care team serving seniors in a value-based model.

You will work with physicians, care coaches and other specialists to deliver comprehensive, high-quality care, manage referrals and coordinate hospital/SNF transitions, with on-call rotation and a focus on TB screening compliance as part Humana's program.

Qualifikationen

• Must hold an active, unrestricted Nurse Practitioner or Physician Assistant license in the appropriate state.
• This role involves patient-facing duties and TB screening.
• Bachelor's degree and clinical leadership experience are required.

• Provide comprehensive senior care following clinical standards.
• Collaborate daily with your care team for coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support center growth plans.
• Deliver value-based care as part of the team.
• Report to the Area Medical Director.
• This is an in-person role at our outpatient clinic.
