This role requires an NP/PA license, a Bachelor's degree, and 8+ years of technical experience with 2+ years in leadership. On-call duties are part of the role, with a 40-hour work week and a compensation range of $111,400–$153,300 per year.

Qualifications

• 8+ years of technical experience in relevant field.
• 2+ years of project leadership experience.
• Active, unrestricted Nurse Practitioner/PA license in the appropriate state.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support center growth plans.
