Job summary

CenterWell Senior Primary Care in Kentucky is seeking a Primary Care Physician to join our team full-time. This role focuses on adult and geriatric outpatient care within a collaborative, patient-centered environment and is aimed at improving outcomes for older adults.

Our team-based care model offers more time with patients, a supportive culture, and a standard schedule Monday–Friday, 8:00 AM–5:00 PM, with compensation including a sign-on bonus, relocation assistance, and comprehensive benefits.

Qualifications

MD or DO from an accredited medical school.
Active, unrestricted medical license in the state of practice.
Board Certification in Family Medicine, Internal Medicine, or Geriatrics.
Must be willing to participate in quarterly on-call coverage by phone.
Proficiency with electronic health records and digital documentation.
2 years' experience managing high-acuity geriatric patients.
Supervise Advanced Practice Providers (NPs) in your assigned clinic.

Responsibilities

You will report to the Associate Medical Director: Dr. Cheryl Mcclain.
You will work fully onsite at a CenterWell clinic in Frankfort or Lexington, KY.
Deliver comprehensive outpatient care to adult and senior patients.
Maintain accurate medical records and documentation.
Diagnose and manage moderately to complex medical conditions.
Coordinate referrals and collaborate with specialists.
Participate in clinical quality improvement programs.
Work with interdisciplinary teams to ensure holistic care.
Contribute to strategic programs and innovations in care delivery.
Exercise independent clinical judgment in patient management.
