Job summary

Rising Ground, Inc. seeks a Preventive Case Planner Supervisor for the Family Connections program in the Bronx. This full-time role reports to the Director, Family Connections (Bronx) and will start immediately.

The Supervisor will coach case planners, ensure fidelity to the Family Connections timeline, and maintain high-quality services for families. The position requires strong supervisory skills, teamwork, and effective planning in a community setting.

Qualifications

• MSW, or other related mental health master’s degree and 3 years post master’s experience at a Preventive Service agency or 5 years’ experience at a human service agency preferred.
• Bachelor’s Degree and 5 years’ experience at a human service agency Required.
• Excellent interpersonal and relationship skills.
• Minimum three years of demonstrated successful supervisory experience preferred.
• Ability to work effectively as a member of a team.
• Good planning, organization and priority-setting skills,
• Demonstrated ability to work with individuals of diverse backgrounds and lifestyles.

Responsibilities

• Supervise teams and provide coaching to case planners.
• Ensure that one hour of change-focused intervention is conducted weekly with each family.
• Review progress notes within 5 business days and ensure fidelity to the Family Connections template.
• Conduct weekly group and individual supervision sessions.
• Ensure timely submission of documentation and adherence to internal timelines.
