Rising Ground seeks a Case Planner to provide direct case planning for children in foster care, connect families with needed services, and support reunification efforts with the child. This is a full-time role based in New York City and requires in-person work at least three days per week.
You will maintain mandated contacts, develop service plans, document progress with reports such as FASP and PH, and participate in Family Court hearings and six-month reviews. Bilingual skills are a plus.

Case Planner provides constructive services to children in foster care to improve their functioning, connects parents to services addressing family challenges, and assists them with reuniting with their child.
Full‑time position. Works Monday through Friday in Brooklyn or Bronx. Work hour options: 9:00 am-5:00 pm, 9:30 am-5:30 pm, 10:00 am-6:00 pm.
