Job summary

Rising Ground seeks a Case Planner to provide direct case planning for children in foster care, connect families with needed services, and support reunification efforts with the child. This is a full-time role based in New York City and requires in-person work at least three days per week.

You will maintain mandated contacts, develop service plans, document progress with reports such as FASP and PH, and participate in Family Court hearings and six-month reviews. Bilingual skills are a plus.

Qualifications

  • Bachelor’s Degree and knowledge of the child welfare system.
  • One year of experience (preferred).
  • Good verbal and written communication skills.
  • Computer proficiency and knowledge of Connections.
  • Bilingual (English/Spanish) is a plus.

Responsibilities

  • Provide direct case planning services that promote child safety, wellbeing, and permanency.
  • Maintain mandated contacts with caretaker, child, parents, siblings, clinical and medical staff, school, and other resources.
  • Develop service plans and connect children or parents to appropriate services that address individual needs.
  • Document progress notes and complete timely and accurate reports, including FASP, PH reports, and intra-agency reporting.
  • Make home visits and collateral contacts to meet ACS and OCFS requirements.
  • Attend Family Court hearings.
  • Facilitate and actively participate in six-month Service Plan Reviews and Family Team Conferences.
  • Establish a culturally appropriate network with other systems within the community.

About the Role

Case Planner provides constructive services to children in foster care to improve their functioning, connects parents to services addressing family challenges, and assists them with reuniting with their child.

What You’ll Do
  • Provide direct case planning services that promote child safety, wellbeing, and permanency.
  • Maintain mandated contacts with caretaker, child, parents, siblings, clinical and medical staff, school, and other resources.
  • Develop service plans and connect children or parents to appropriate services that address individual needs.

Additional Requirements
  • Authorized to work in the U.S.
  • Ability to work in person in New York City for at least three days per week.
  • Ability to travel to other Rising Ground sites if required.
Position Details

Full‑time position. Works Monday through Friday in Brooklyn or Bronx. Work hour options: 9:00 am-5:00 pm, 9:30 am-5:30 pm, 10:00 am-6:00 pm.
