Qualifications

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and aides in resident care.
• Communicate with physicians, nursing staff, and families regarding progress and discharge plans.
• Maintain timely, accurate documentation compliant with facility and payer requirements.
