Job summary

Meadow Park Rehabilitation & Healthcare Center in New York is seeking a dedicated Full-Time Physical Therapist to join our rehabilitation team and provide skilled PT services across short-term and long-term residents.

The ideal candidate holds a NY State PT license, demonstrates strong clinical judgment, and collaborates with physicians, nursing staff, and interdisciplinary team members to develop and monitor individualized treatment plans.

Qualifications

• Current New York State Physical Therapist license in good standing.
• Strong clinical, communication, and organizational skills.
• Skilled nursing facility experience preferred, but not required.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled physical therapy services to short-term rehabilitation and long-term care residents.
• Monitor resident progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, and interdisciplinary team members.
• Maintain timely and accurate documentation in accordance with facility and regulatory requirements.
• Educate residents and families regarding treatment goals and therapeutic programs.
