Engage Senior Therapy is seeking a dedicated Physical Therapist to join our team in Warner, NH, delivering compassionate care at assisted living facilities. You’ll evaluate patients, develop personalized treatment plans, and provide therapies aimed at improving mobility and independence for seniors.

Qualifications

• Graduation from an accredited physical therapy program with an active or pending state PT license.
• Passion for geriatric care and improving independence for seniors.
• Strong interpersonal skills to collaborate with patients, families and staff.

Responsibilities

• Evaluate patients to assess strength, balance, mobility and functional impairments.
• Develop and implement personalized treatment plans focusing on mobility and pain reduction.
• Deliver care at assisted living locations with independence and supervision from Engage team.
• Provide therapies such as manual therapy, therapeutic exercises, gait training and balance programs.
• Educate patients and families on home programs to maintain functional independence.
• Document care in EMR compliant with state and facility standards.
