Qualifications

• Graduate of a CAPTE‑accredited Physical Therapy program.
• Active state PT license in good standing (or eligibility).
• Commitment to compassionate, evidence‑based geriatric care.
• Strong communication and interpersonal skills with a collaborative mindset.

Responsibilities

• Conduct comprehensive evaluations of strength, balance, mobility, pain, and function.
• Develop and implement individualized treatment plans to improve mobility and independence.
• Deliver hands‑on interventions including therapeutic exercise, balance and gait training, and manual therapy.
• Promote safety and fall prevention through functional assessments and education.
• Maintain documentation and progress in EMR systems per state and facility standards.
• Collaborate with therapists and care teams for coordinated patient care.
• Participate in mentorship and continuing education opportunities.
• Build relationships with residents, families and staff to support outcomes.
