Qualifications

• Requires Bachelor's/Master’s/Doctorate in Physical Therapy.
• Current state license is required.
• Strong oral and written communication abilities.

Responsibilities

• Screen, evaluate, and treat facility patients/residents needing Physical Therapy services.
• Review medical information and conduct screenings to determine need for services.
• Perform evaluations under physician order and develop treatment plans.
• Implement treatment plans through direct treatment and staff education.
• Supervise Physical Therapy Assistants and rehab aides per state laws.
• Attend interdisciplinary meetings to relay findings and recommendations.
• Attend family meetings as appropriate.
• Maintain accurate billing information and documentation.
• Ensure compliance with facility policies and Rehab procedures.
• Provide staff training and in-services as needed.
• Follow confidentiality policies for patient, facility, and physician data.
