Job summary

Bedrock Health Care Center in Madison, WI seeks a Physical Therapist to evaluate and treat residents with impairments that affect independence. You will document care and participate in an interdisciplinary team to support residents' functional goals.

Qualifications

• Excellent verbal and written communication and organizational skills.
• Ability to read, analyze, and interpret common medical information.
• Familiar with Word and Excel.

Responsibilities

• Evaluate and document evaluations of referred residents per facility policies.
• Provide treatment to help residents maintain or attain the highest level of function.
• Recommend and/or provide appropriate treatment and adaptive equipment to prevent decline.
• Maintain timely and accurate resident reports for regulatory agencies and third-party payers.
• Observe infection control procedures.
• Be responsible for the safety of residents under supervision.
• Complete documentation related to evaluations, treatment plans and MDS assessments in a timely manner.
• Participate in resident care conferences and rehabilitation conferences.
