Job summary

Responsibilities

• Leads and oversees outreach to members via phone or home visits to engage member and discuss care plan/service plan.
• Coordinates care activities based on the care plan/service plan with providers and caregivers.
• Provides continuous support for complex member or provider inquiries related to care plans.
• Documents, maintains, and reviews member records per state and regulatory requirements.
• Triages, adjusts, and escalates highly complex requests to management.
• Assists with training and development needs as required.
