Responsabilidades

• Provides outreach to members via phone to support care plan steps and scheduling.
• Supports members to connect them to health plan resources for high-quality care.
• Acts as frontline support for member/provider inquiries and explains care plan procedures.
• Assists with member onboarding and day-to-day administrative duties, including sending welcome letters and program materials.
• Documents non-clinical member records per state and regulatory requirements.
