Job summary

Beth Israel Lahey Health is seeking a Nurse Navigator to serve as the first point of contact for patients, guiding them through the care continuum from diagnosis onward. The role coordinates between PCPs, surgeons, oncologists and other providers while educating patients and families about diagnoses and treatments.

The Nurse Navigator will work with Radiology and Surgery teams, oversee education for patients and staff, and participate in quality and data analysis.

Qualifications

• Bachelor's degree required.
• Registered Nurse (RN) license required.
• American Heart Association Basic Life Support (BLS) certificate required.
• 3–5 years related clinical medical-surgical experience.
• Strong analytic and data analysis skills.
• Excellent interpersonal and collaboration abilities.
• Experience with computer systems including MS Office applications.

Responsibilities

• Initiates communication with patients after a positive finding and guides them through the health care system.
• Provides health education to individuals and groups and develops learning materials.
• Identifies department personnel involved in care and develops relationships with them.
• Assists with questions about payment for services and arranges patient transportation when needed.
• Tracks interventions and outcomes and participates in medication reconciliation during care transitions.
